fender shaving is OK, but will this size need any more modifications than simply shaving ?

19 x 10 via 30 offset on the rear of a 2001 LS430

car is stock height and NOT lowered.

if the car isnt lowered you could probably fit 20x12 -35

On the contrary, stock height means the IRS is putting the wheel further out than when it is lowered. Even 19x10+30 might stick out slightly when looked from the side. I have a booklet showing what you can do at various heights for different cars... will look when i get home.
